Catalyst Physical Therapy & Diagnostics (“Catalyst,” “we,” “our,” or “us”) respects your privacy. This Privacy Policy explains how we collect, use, protect, and share personal information through our website, forms, communications, and services.
This policy applies to information collected through our website, appointment-request tools, online chat, email, telephone, text messages, and other interactions with Catalyst.
Healthcare, treatment, billing, insurance, and patient-record information may be protected by HIPAA and other laws. This Privacy Policy does not replace Catalyst’s Notice of Privacy Practices, which explains how we use and disclose protected health information (“PHI”) and describes related patient rights. Until our Notice of Privacy Practices is posted online, you may request a copy by calling (727) 286-8408 or asking our office.

When you visit our website, we may automatically collect technical and usage information, including your IP address or approximate location, browser and device information, pages viewed, links selected, referring website, date and time of visits, cookie or advertising identifiers, interactions with our advertisements, online-chat submissions, and call-routing or call-tracking information.
Please do not send detailed medical, insurance, financial, or other confidential information through a general website form, ordinary email, text message, or online chat unless Catalyst directs you to use a secure method.
Website Tracking and Advertising
Catalyst uses Google Analytics, Google Tag Manager, Google Ads, call-tracking services, online chat tools, and similar technologies to operate and improve our website, understand website traffic, respond to inquiries, measure advertising effectiveness, and improve the visitor experience.
These technologies may use cookies, pixels, scripts, log files, or similar tools to collect technical and website-usage information. Catalyst does not intentionally provide names, email addresses, telephone numbers, appointment details, treatment or insurance information, medical-record information, or other PHI to website analytics or advertising providers.
Online chat tools collect information you choose to submit. Call-tracking tools may use unique telephone numbers or similar technology to measure how website visitors contact Catalyst and assess advertising performance.
Catalyst may use Google Ads and other advertising/measurement technologies to measure, evaluate, or help deliver advertising. Where required by law, Catalyst will provide applicable notice, consent, or opt-out choices.
You may manage cookies through your browser settings. You may also manage certain Google advertising preferences through Google Ads Settings or limit Google Analytics data collection through the Google Analytics Opt-out Browser Add-on.
